Absence Report
If your child will be or has been absent, please submit the absence form. You’ll need your child’s name, grade, class, dates of absence, and reason.

Emergency Contact Form
Keep your emergency contact information up to date. If you’re late picking up your child, the school will contact your emergency contacts. In case of illness or building emergencies, having current information is critical.
You can complete a physical blue card at school or use the online form.
OMNY Card Request
Eligible students receive OMNY tap-to-go transit passes providing 4 free rides per day, year-round including weekends and summer.
Student Verification Letter
Request an official letter verifying your child’s enrollment. All requests are filled within 48 hours. A copy will be sent home with your student unless you choose a different option.
Families will be contacted if their student has below 85% attendance.
